migratory bird survey

does any one have the address to mail in the migratory waterfowl form to. My son has been filling this out lost the envelope, i called the number at the bottom of the form voice mail and no reply back.i would hate to think this form is a waste i also relise its late.

This harvest data is very important for sound management of waterfowl populations, and we greatly appreciate the hunters who take the extra time to cooperate in these surveys. The information is still valuable and will be used to estimate the 04-05 season harvest. You can mail the survey to the address below:

Diane, is there any reason these surveys cannot be made available to all who purchase a Florida duck stamp or Sportsmans license? In 4 years I have never received one.
